When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access:
- Collect a consent form from reception between 10am to 6.30pm
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Online access for children
- You can also have management over your children’s online access enabling you to arrange appointments, order prescriptions and view their medical records on their behalf.
- When your child turns 13, your online access will automatically be removed due to “Gillick Competency”.
- Your child can give the practice consent to reactivate your access if this is their wish after an assessment by a clinician. Please contact the practice to arrange this assessment.

- If your child should wish to manage their own online access at the age of 13 then the practice can provide them with their log in details.
Managing your health online
Using an online form
There are many things you can do online at our surgery. Some tasks you can do using a website called [PATCHS LINK] where you can:
- Request an appointment
- Request a sick note
- Ask about test results
- Contact us about an admin request
- Send information such as blood pressure readings, consent forms etc
Using your NHS account or Airmid
There are other tasks you can do online, like:
- order repeat prescriptions
- view your test results
- see parts of your health record
- check or cancel appointments
- see your vaccination record
For these, you can use your NHS account or Airmid. You can access them using their websites or apps.
Setting up an NHS account
To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).
Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.
Setting up Airmid
To set up Airmid for the first time:
- Visit the surgery with photo ID so we can confirm your identity
- We will give you a username and password
- Go to ADD LINK or use the app to register
